Periodontal flap surgery creates controlled access to the tooth roots and supporting bone so deposits, inflamed tissue and difficult-to-reach areas can be treated directly. It is not the first treatment for every case of bleeding gums. Many patients improve with professional scaling, oral-hygiene changes and scaling and root planing. Surgery becomes more relevant when deep periodontal pockets remain, bone contours need to be assessed or corrected, or access is needed for regenerative or other periodontal procedures. The exact approach depends on pocket depth, bone loss pattern, tooth mobility, furcation involvement, plaque control, smoking, diabetes control and the long-term prognosis of the teeth involved. Some procedures aim mainly to reduce pocket depth, while others may include regenerative materials, crown lengthening or management of specific gum defects. Healing and final gum position vary between patients, and teeth can appear longer after inflammation is controlled or when the gumline changes during treatment. Long-term periodontal maintenance remains essential after surgery because periodontitis is a chronic disease and can recur if plaque control and risk factors are not managed.
